Self-Certification
If you have fallen ill and will be off for seven days or less, you do not need a sick note. However, your employer may request you to complete a self-certification form. This form can be obtained from either your employer or from this HMRC page here.
Requesting your first sick note
If your illness remains after the self-certification period, you can request a sick note from the GP. You would need to have a telephone consultation.
Repeat Fit Note Requests
Please allow up-to 48-hours between requesting and collecting the repeat fit-note
Repeat fit-notes/ sick-notes can now be requested by using the links provided below. Unless you would like to speak to a GP, you do not need to book a follow-up telephone consultation.
Fit notes will be sent to you by email when created. Please make sure your email is up to date on our system by either checking the NHS App or speaking to our reception team.
